I'm trying to download the provided csv template in order to add a test email to an advanced outreach I'm testing, and the csv does not appear to be configured correctly.

I tested this in a demo org and didn't see the same issue that you are experiencing. I'd recommend that you shoot an email to support@gainsight.com and get a tech to work with you one on one to troubleshoot this issue. Might be a local environment related root cause.
