In-person delivered survey tied to specific contacts

We have our customer conference coming up and are expecting upwards of 1500+ customer contacts at the event. Our Content Curation (self help resources) team is looking to administer a survey at the event to assess the perceived quality of our self help resources.

We are looking for a way to have a survey ready for customers to fill out that will still tie back to their specific contact. We would like to avoid having to send them an email. All customers will have badges that can be scanned and contain their email. We'll be sure to have an iPad on hand for them to fill out the survey.

Any ideas?

Hi Ben , 

Just checking if its resolved , if its not you can follow these steps with survey for workaround  :-

So you can design an anonymous survey and put a mandatory text box question in survey itself asking the contact info of the user .Then you can generate the link for survey and send that link by slack , text message , hangout etc.This way while answering the survey user can fill there contact detail  and you will have the data.
