I have a user who shows 23 PMs on the drop down
but on his Private Message page, it only shows 2 messages. (second user name I blocked for privacy)
Am I missing something? Is there a way a user can delete messages?

Nothing is missing.
There you have two conversations.
The count of PMs is NOT the number of conversations, but the number of individual messages sent TO the user.
For example.
I have 15 conversations in my PM list, with 31 being the count.
Hopefully that clears it up! 😀
