We trigger a survey to be sent to attendees after a certain activity is logged. We would like to associate the survey responses to the activity that triggered it for reporting purposes. Is there any way to do this?

As Timeline Activity does not connect with the other object/s you need (eg, NPS Survey, the flattened object of the survey itself) you can’t use Report Builder, you have to leverage Data Designer for this.
Depending on the data you want to get, you will need to play with the merge/s filter/s. But overall you are looking at a merge of Activity Timeline filtered by Activity Type X, with the source you want to use for your survey data where the Company GSID matches, for example:
