Community Health Survey

Hey, we are looking to create a community health survey and I am wondering if anyone has any insight or examples they’d be willing to share? Essentially, we are looking for feedback on the current state of the community as well as looking forward and asking what they would like to see from the community in the future.

Hi Erin! Do you know about the Sense of Community Index? If you run this same survey every 12 months you’ll have a great baseline of annual information to track (though I wouldn’t run it more than once a year. I’m sure there are other survey efforts happening in your community and you don’t want to give your members survey fatigue.).
