Has anyone experienced in data designer when trying to create a formula field, no matter what field you choose it inputs a different field? In the attached screenshots I am trying to use a field called renewal uplift (2nd screenshot), but once i click it, its choosing a completely different field (first screen shot)

It is an issue with the working of the formula field in the Data Designer. I recommend you raise a support Ticket - https://gainsight.zendesk.com/ and the team will help you with a workaround or fix.
