Capture banned reason and who banned them

When banning a user from the community it would be helpful to have a way to record the ban reason somewhere which would stay with the profile for future context and team members. Sometimes someone is banned, but those details are lost or they may even contact someone else on the team and others would have no idea as to why or even whom banned this person.
