Inline survey not populating in JO

I have created a Generic Survey and added the survey to a template using the “question” survey type. When I go to add this survey to my JO program, I cannot select the ‘question’ in the set up of JO. Please let me know what i am doing wrong! thank you!

One hypothesis: The checkboxes in front of the 1-5 scores has me wondering if your question is eligible to appear on the list of in-line surveys. I’d likely start by checking that my question is of the configuration that will allow it to appear in the in-line list.

Only single-select questions on the first page of the survey will be available for selection.
