I know that we can add an email every time an outreach runs.. but is there a way to receive a notification if an outreach fails to run?

Are you working with scheduled outreaches? If so, you can use the mail icon in the right corner of the scheduler to enter an email address for the outreach results to be sent to. This should send results for both successes and failures. The mail icon is highlighted here.
