Make Success Plans Linkable from the Cockpit and CTAs

We currently can add the Success Plan name (if assigned) to the list view of the cockpit as a text field. The suggestion is to make this a linkable field that takes the user directly to the success plan if clicked from the cockpit list view. 

The second suggestion is to add a field on the CTA that references the success plan (if there is one) and has the same action.
