I recently ran into a survey link issue with a customer and had an idea that may help with these in the future. For a survey link to work properly, it is my understand that the Account ID and Contact ID have to be valid for the link to build correctly. In my case, we were sending surveys to Community Users in SFDC and being pulled from the User object, not contact. User ID was mistakenly mapped to the Contact ID field in the participant mapping. It would be great if we could validate that the field really is a contact ID (maybe must start with '003'). This would better prevent that surveys don't get sent with bad links in the future.
